I work in a community hospital which sees a variety of patients. Our floor
recently had as a charge a 7 y/o girl with neutropenia secondary from
chemotherapy treatments. Our hospital does not care for this level of
patients rountinely. A question was posed on what reverse precautions were
and what the difference was from compromised host or neutropenic
precautions. Some staff members used universal precautions while others
used gown/gloves/mask as their standard precautions. What are other
hospitals policies with these cases?

-My experiences with neutropenic isolation are as follows: No flowers, uncooked foods (fruits, salads, raw veggies, etc.)
allowed in pt. room.
Mask required to enter room, gloves when in contact with pt.
Positive pressure room if possible.
CMV irradiated blood if needed.
Limit venous punctures.
Inform physician if temp.>100.4F
Pt. to wear mask if allowed to leave room. I think that is it.
Obviously, great care must be taken with these pts. The above must be
followed carefully and early detection of infection is crucial for a
good outcome.
